We have an open list and generally welcome requests for registration from patients living in or moving to the practice area. It is simple and easy to register with us;
1) Check you live within the Practice Boundary
You can view the Practice Boundary online here.
Anybody living with the Practice Boundary has an expressed right to register with the pracitce, unless they have been previously removed from the practice live. If you don't live within the practice boundary, but do live within a PR5, PR6, PR7, PR25, PR26 or BL6 5HL post code you may still be eligible to register, but this will depend on your individual circumstances. 2) Check If You Take The Following Medication Before Registering
We are unlikely to be able to prescribe your medication immediately and will need to wait for your clinical record to arrive on our system. You may also need to see a clinician. Please ensure you have a reasonable supply of medication from your previous GP before registering with us.
Any new patients registering with us who are currently prescribed hypnotics (sleeping medication) or anxiolytics (anti-anxiety medication) included in the list below may be required to participate in a withdrawal regime unless a G.P. decides this is not clinically appropriate or if being prescribed by secondary care:
- Diazepam
- Loprazolam
- Lorazepam
- Nitrazepam
- Oxazepam
- Temazepam
- Zolpidem
- Zopiclone
By registering with the practice, you agree to participate in a withdrawal regime from the medication stated above unless a G.P. decides this is not clinically appropriate.
If you are currently being prescribed stated below by Secondary Care and are being monitored AND prescribed the medication by a secondary care service only. Should your circumstances change, please note Library House Surgery WILL NOT prescribe or monitor the medication below, under any circumstances.

3) Submit your registration forms
The easiest and quickest way to do this is via the NHS Register With A GP Service.

4) We will confirm your registration
We will process your registration form and check you are eligible to join the practice list. Once completed we will contract you to confirm your registration. This process can take up to 48 working hours.
You may be invited for a new patient health check as part of the process.
You will be registered as a patient with the Practice, rather than with an individual doctor, however you can request a specific doctor if you need an appointment which we will always try to accommodate.
