Alternatively, you are able to collect one of these forms from main reception.
If you do not have your own blood pressure machine, we do have a limited number of machines we can loan to patients. When loaning a machine we expect:
- The equipment is loaned to you for a set period and must be returned by the agreed date. A £10 deposit is required.
- The equipment is only for the named patient and must be used as instructed.
- We check that the equipment is clean, safe, and working properly before you take it home.
- Do not open, alter, or repair the equipment. Please do not change the batteries — return it to the Surgery if there is a problem.
- You must return the equipment on time, along with a copy of the agreement, and ask reception to confirm it has been returned. If it is not returned, your deposit will be lost, and further action may be taken.
- Loan periods can only be extended with written approval from the Practice.
